Individuals that are age 65 or older, blind or disabled may apply for medical assistance at:

Who to contact if you have lost or not received your ProviderOne Services Card:

There are several ways to request a replacement:

• Visit the ProviderOne client portal website: https://www.waproviderone.org/client

• Call the toll-free IVR line at 1-800-562-3022, follow the prompts.

• Request a change online: https://www.waproviderone.org/client

• Select the topic "Services Card."

There is no charge for a new card. It takes 7-10 days to get the new card in the mail. Your old card will stop working when you ask for a new one.
